The Department of Public Health (DPH) is recruiting for Public Health Nurses I (PHN I)who provide assessments of individuals, families and communities.
Duties include intensive case management, coordinating and assisting with access to care; developing treatment plans; providing consultation on medical, dental and behavioral health assessments; acting as a nurse liaison with public and private agencies, providing assessments and referrals for school district students; and conducting disease control measures.
Vacancies exists throughout Family Health Services (FHS) and California Children's Services (CCS) sections in DPH.For more detailed information, refer to the Public Health Nurse I job description.
Incumbents in this classification are eligible for promotion to Public Health Nurse II upon successful completion of a trainee period and satisfactory work progress report.
Human Resources is also accepting applications for Public Health Nurse II. A separate application is required. It is to your advantage to apply for both levels as appropriate.
Family Health Services
Family Health Services (FHS) section of DPH specializes in many dynamic public health nursing programs and includes Health Care Program for Children in Foster Care, Rx 4 Kids, Children Screening, Assessment, Referral and Treatment, Black Infant Health, and Maternal Child and Adolescent Health.
California Children's Services
The California Children's Services (CCS) section of DPH is a statewide program that arranges, directs, and pays for medical care, equipment, and rehabilitation, when these services are authorized for children and young adults under 21 years of age, who have CCS-eligible medical conditions and whose families are unable to pay for all or part of their care.

CONDITIONS OF EMPLOYMENT
Travel/Driver License:
Travel throughout the County is required. Employees will be required to make provision for transportation.
At the time of hire, a valid California Class C driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.
Pre-Employment Process:
Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.
Visa Sponsorship:
Please note San Bernardino County is not able to consider candidates who will require visa sponsorship at the time of application or in the future.
Continued Education:
Additional training/coursework may be required.
Trainee Period:
Public Health Nurse I must be promoted to journey-level classification upon successful completion of the trainee period (e.g., preceptorship, orientation, and/or certification, as applicable), or be terminated.

RNs mustpossess a bachelor's degree (or higher) in Nursing to apply for California Public Health Nurse certification.
Applicants who do not possess a Bachelor's degree (or higher) in Nursing at the time of application submission will be disqualified.

Experience working with pediatric populations which includes enhanced care management experience, certificate or training and the ability to use social determinants of health in developing care plans and interventions is highly desirable

Additional compensation is associated with positions requiring the use of bilingual skills. Testing of bilingual skills may be required to receive additional compensation.
