    The Department of Public Health (DPH) is recruiting for Public Health Nurses II (PHN II)who provide assessments of individuals, families and communities.

    Duties include case management, coordinating and assisting with access to care; developing treatment plans; providing consultation on medical, dental, and behavioral health assessments; acting as a nurse liaison with public and private agencies; providing assessments and referral for school district students; and conducting disease control measures.

    There are vacancies in Family Health Services (FHS) and California Children's Services (CCS) sections in DPH.

    The eligible list established from this recruitment will be used to fill vacanciesin the High Desert only.
    Candidates who do not indicate a willingness to work in Victorville, Hesperia, and Barstow will be disqualified.

    Human Resources is also accepting applications for Public Health Nurse I. A separate application is required. It is to your advantage to apply for both levels as appropriate.

    Family Health Services


    Family Health Services (FHS) section of DPH specializes in many dynamic public health nursing programs and includes Health Care Program for Children in Foster Care, Rx 4 Kids, Children Screening, Assessment, Referral and Treatment, Black Infant Health, and Maternal Child and Adolescent Health.

    FHS works in partnership with public and private agencies to provide nursing case management, care coordination and consultation.

    California Children's Services


    The California Children's Services (CCS) section of DPH is a statewide program that arranges, directs, and pays for medical care, equipment, and rehabilitation, when these services are authorized for children and young adults under 21 years of age, who have CCS-eligible medical conditions and whose families are unable to pay for all or part of their care.

    CCS determines eligibility and authorizes qualifiedprofessionals to treat the child's CCS eligible condition.

    • Recruitment Bonus: Eligible employees hired into a regular classified position from this recruitment may be eligible to receive $3,000 in total compensation. Please seeCNAMOUfor details.
    • Recruitment Relocation Assistance:Newly hired employees from this recruitment may be reimbursed up to $2,500 for relocation-related expenses upon meeting eligibility criteria. Please see CNA MOU for details.
    • Telework Program:The County allows for the use of telework to qualifying County employees where business needs permit to efficiently serve the community and maintain employee productivity during public health emergencies. Candidates, upon successful completion of a probationary period, may potentially have the option to telework based on their assignment.

    CONDITIONS OF EMPLOYMENT

    Travel/Driver License:
    Travel throughout the county is required. Employees will be required to make provision for transportation.

    At the time of hire, a valid California Class C driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.


    Pre-Employment Process:
    Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening

    Continued Education:
    Additional training/coursework may be required.

    Trainee Period:

    Public Health Nurse I must be promoted to journey-level classification upon successful completion of the trainee period (e.g., preceptorship, orientation, and/or certification, as applicable), or be terminated.

    Public Health Nurse Certification:
    RNsmustpossess a bachelor's degree (or higher) in Nursing to apply for California Public Health Nurse certification.
    Applicants who donotpossess a Bachelor's degree (or higher) in Nursing at the time of application submission will be disqualified.

    Minimum Requirements

    Applicants must meet bothof the following licensure/certification -and- experience requirements:

    Licensure/Certification:
    Applicants must possess and maintain all of the following items:


    • A valid Registered Nurse (RN) license issued by the California Board of Registered Nursing (CA BRN),
    • A valid Public Health Nurse (PHN) certificate issued by the CA BRN,
    • A valid Healthcare Provider Basic Life Support (BLS) certificate.
    * AND-Experience:Applicants mustmeet one of the following experience options:


    • Option 1:Three (3) months of full-time equivalent experience as a licensed Public Health Nurse.


    • OR-

    • Option 2:Three (3) months of full-time equivalent experience in home health or home visiting as a licensed Registered Nurse.
    Desired Qualifications

    CCS Section:

    Experience working with pediatric populations which includes enhanced care management experience, certificate or training and the ability to use social determinants of health in developing care plans and interventions is highly desirable.

    Applicants fluent in American Sign Language, Arabic, Korean, Mandarin, Spanish and/or Vietnamese are strongly encouraged to apply. Additional compensation is associated with positions requiring the use of bilingual skills. Testing of bilingual skills may be required to receive additional compensation.

    Candidates possessing computer skills in Microsoft applications are preferred.
