    POSITION SUMMARY/JOB PURPOSE:

    The CSC (Coordinated Specialty Care) Program Coordinator guarantees that services are delivered as required for the Coordinated Specialty Care First Episode Psychosis (CSC FEP) program. This program caters to individuals aged 15 to 30 facing their initial onset of a psychotic disorder. The team collaborates weekly with individuals to aid them in their job or educational needs while supporting them in comprehending their mental health and striving towards recovery objectives. The CSC Program Coordinator oversees and monitors service provision, maintains track records, submits reports, and ensures clinical meetings are conducted. This role involves directly supervising the CSC Case Manager and collaborating closely with supportive employment specialists, family partner specialists, peer support specialists, and medical personnel. The CSC Program Coordinator also delivers direct services to individuals in the program including providing skills training, case management, psychoeducation, and counseling services.

    This role operates independently with minimal oversight, reporting major activities through regular meetings.

    EDUCATION, EXPERIENCE, OTHER QUALIFICATIONS:

    Education Required: Preferably, a Master's Degree in Counseling, Psychology, Social Work, or a related field with full licensure as an Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT). Applicants with professional licensure status of LPC-Associate, LMFT-Associate, or LMSW under clinical supervision will also be considered.

    Experience Required: Minimum of 1 year of experience in social services with at least 1 year of preferred supervisory experience.

    Registration, Certification, Licensure or other Qualifications Required:

    • Valid Texas Driver's license, auto liability insurance, and an acceptable driving record as per PermiaCare's insurance requirements are essential
    • Criminal history and background checks clearance, as well as passing a pre-employment drug screen are compulsory
    • Attainment of QMHP (Qualified Mental Health Professional) certification within 6 months post-hiring

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Provide direct services to individuals specified in the recovery plan and TRR assessment
    • Supervise the CSC Case Manager directly
    • Oversee services provided by Supportive Employment Specialists, Family Partner, and Peer Support Specialists
    • Conduct weekly CSC team meetings
    • Hold bi-weekly meetings with supportive employment specialists
    • Track monthly performance metrics
    • Submit quarterly reports to Texas Health and Human Services Commission (HHSC)
    • Offer education and outreach regarding FEP to individuals in services, family members, the community, and local stakeholders
    • Connect individuals in services with necessary integrated services such as medical care, substance abuse treatment, crisis services, veteran services, or other crucial services as identified by clients
    • Assist CCBHC Manager in maintaining workflow and operations to ensure effective services that meet client needs
    • Achieve unit performance targets
    • Maintain a caseload of individuals with mental illness
    • Coordinate services for designated caseload
    • Ensure timely and accurate documentation of services
    • Maintain confidentiality of sensitive records and treatment information in compliance with HIPAA regulations
    • Ensure regular and reliable on-site attendance, demonstrating dependability, and punctuality
    • Adhere to the Abuse, Neglect, and Exploitation policy
    • Adhere to the Code of Conduct and Standards of Behavior
    • Establish and nurture effective work relationships with individuals served, their families, supervisors, co-workers, and visitors
    • Communicate consistently with supervisor
    • Process mail and email promptly
    • Answer phone calls, take messages, and respond to inquiries promptly and accurately
    • Maintain a safe and tidy working environment
    • Work using a trauma-informed approach
    • Portray professionalism when representing PermiaCare and the program externally
    • Maintain compliance with legal requirements and company policies
    • Ensure possession of a valid driver's license, auto insurance, acceptable driving record, and reliable transportation at all times
    • Complete all assigned training on time
    • Perform any other duties as assigned

    MARGINAL DUTIES AND RESPONSIBILITIES

    • Fill in for other MH staff as required
    • Ensure progress notes accurately reflect progress according to current provider agreement
    • Complete discharges and discharge summaries as necessary and within regulatory guidelines
    • Identify client needs and risks through TRR assessments and collaboration with relevant teams
    • Participate in community activities and meetings as needed
    • Attend workgroups and committees as assigned
    • Provide translation services if applicable
    • Participate in team meetings
