- Friday, 8am to 5pm

What You Will Do
We are seeking a dedicated Nurse Practitioner to join our Longitudinal Care team in Tempe, AZ.
As a Nurse Practitioner in Longitudinal Care, you will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey.
This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care.- Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
- Diagnosis and Treatment: Formulate accurate diagnoses and develop individualized treatment plans based on evidencebased practices and guidelines.
- Chronic Disease Management: Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
- Medication Management: Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.
- Preventive Care: Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
- Patient Education: Provide patient education on disease management, healthy lifestyles, and selfcare practices to empower patients to take an active role in their health.
- Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
- Documentation: Maintain accurate and uptodate electronic health records (EHR) to track patient progress, treatment plans, and interventions.
- Follow-Up: Schedule and conduct regular followup appointments with patients to assess treatment effectiveness and make necessary adjustments.
- Quality Assurance: Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
Education and Experience:
- Holds active unencumbered license in Arizona
- Board certified with AANP or AANC
- Has an active DEA license
- Current CPR certification.
- Two or more years of practice experience preferred
- Must have valid state driver license and automobile insurance
