Family Services of Westchester is looking for part time-time Job Coach for our Yonkers, NY location.

Job Summary:

provides services to the Westchester County Reentry Task Force (WCRTF) on a part-time basis to offer a credible messenger approach to reentering the workforce after reentering community.


Responsibilities:


  • Plan and implement a job coaching strategy with participants
  • Assess the professional strengths and weaknesses of each participant
  • Discover what motivates participants and assist them in utilizing that motivation in the workplace
  • Develop relationships with employers in the community and educate them about EITC
  • Complete RSW training when offered
  • Recruit for, participate in, and coordinate with the RSW facilitator to offer support to participants and increase likelihood of their completion
  • Assist participants with job search, interviews, transition into and maintaining job
  • Maintain case management case load (up to 5)
  • Facilitate case management between providers and facilities
  • Conduct planning and coordination, regularly interface with parole
  • Develop communitybased provider service plans
  • Coordinate with team and then provide referrals to such needed services as employment readiness programs: MRT, Ready Set Work and others
  • The designated worksites of the Case Manager will include the DCMH offices and DOCCS New Rochelle parole office
  • Maintain detailed case notes, perform programmatic data entry duties and submit weekly records to the Reentry coordinator

For assigned caseload:


  • Complete intakes/assessments and make referrals as requested by supervisor
  • Provide referrals to offender accountability programs: anger management, domestic violence counseling, DWI impact panel, education and employment services, housing opportunities, family reunification, mentoring, and other services that address criminogenic and stabilization needs and are consistent with conditions of parole
  • Distribute and manage metro cards
  • Maintain contact with treatment and other service providers to monitor reentrants' progress and communicate with parole officers and the CRTF team
  • Refer eligible reentrants to all existing Health Homes throughout Westchester County, giving each reentrant the option to select the Health Home of his/her choice

Minimum Qualifications:

Associate's, Bachelor's (preferred), Lived Experience (required),

Case Management:
l year (required), Driver's License (preferred)
