Per Diem or Part Time, Weekdays, Day Shift hours (no weekends, evenings, or overnights) for Home Health Cases


Multiple cases per day

Hawthorne and surrounding areas

NYS HHA Certification Required

HS Diploma or GED, and Valid Driver's License and reliable transportation Required

Pay Range:
$22.00/hr to $26.00/hr

Responsibilities:


  • Provides personal care assistance to patients.
  • Understands basic elements of body functioning and reports changes in patient's body functions as indicated.
  • Plans and prepares nutritious meals.
  • Light housekeeping.
  • Lifts and transfers.
  • Follows specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care.
  • Other duties as assigned.

Qualifications:


  • High school diploma or equivalent.
  • Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
  • CNA experience within the last two years. If it has been more than two years, you must complete another training and competency program before providing services.
  • Must be able to successfully pass the competency program of the care center.
  • Current CPR certification.
