Compass Home Care provides home health care to patients of all ages across five counties. Our multi-disciplinary team includes, nursing, aide, social work and nutrition counseling as well as physical, occupational and speech therapies.

**Job Skills / Requirements**
-Participating in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury
-Maintaining appropriate clinical records, clinical notes, and reporting to the Occupational Therapist any changes in the patients condition
-Submitting clinical documentation
-Following treatment program and goals for improved patient function as established by the Occupational Therapist
-Documenting patients response to treatment plan and progress toward established goals
-Maintaining contact/communication with other personnel involved in the patients care to promote coordinated, efficient care
-Attending and contributing to in-services, case conferences, and other meetings
-Identifying patient and family/caregiver needs for other home care services, consulting with the supervising Occupational Therapist, and assisting with necessary referrals as appropriate
-Participating in instructing patients family/caregiver and other Organization health care personnel in patients treatment regime as delegated by the Occupational therapist
