EDUCATION:

Bachelor's degree required, preferably in Public Health, Biology, Healthcare Administration, or related field. Master's degree are preferred.


WORK EXPERIENCE:
2 years or more in healthcare preferred


SALARY RANGE:

$18-$24/hour


FLSA STATUS:

Hourly - Non-Exempt

JOB SUMMARY:


The Chronic Disease Project Coordinator will be responsible for development, implementation, and evaluation of chronic disease care coordination programs for Hypertension, Diabetes, Hyperlipidemia and other high risk chronic disease patients.

This project development includes a plan for policy and procedure development, outreach activities, patient enrollment into the programs, integration of the program at all clinic sites, patient education material, inventory and purchasing for the programs, as well as data collection.

The chronic disease project coordinator will track patients enrolled in remote patient monitoring for hypertension and diabetes and ensure that follow-up is conducted for these programs.

The Chronic Disease Project Coordinator will also be responsible for ensuring the appropriate documentation and data entry for all patients in the Chronic Disease programs.

Staff will also work on HEDIS and quality goals. This position will coordinate with HOPE Clinic staff and volunteers to ensure program success.


MAJOR DUTIES & RESPONSIBILITIES:


  • Program development in conjunction with clinical and quality staff including policies, procedures, implementation plans, and a plan for monitoring program outcomes;
  • Data collection and presentation at monthly quality meeting to keep leadership informed about program progress and barriers as necessary;
  • Train and educate staff and volunteers on program procedures and goals;
  • Maintain staff/volunteer assignments for chronic disease program enrollment at each site;
  • Train patients on chronic disease selfmanagement using approved materials;
  • Assess patient care gaps within EHR tools and as directed by provider and program director;
  • Competency in front desk, eligibility and scheduling procedures;
  • Provide translation assistance as needed;
  • Answer telephone calls and messages within the same day;
  • Recall patients and follow up on HEDIS measure lists and items;
  • Participate in quality measure projects to improve HOPE outcomes for UDS recall lists, process improvement items and other quality related job tasks;
  • Followup on no show appointments and any missing patient remote patient monitoring data as directed by program protocol;
  • Provide education and training to other clinic staff to help them learn about the chronic disease
- management program at HOPE;

  • Work with other team members and managers to implement daily duties and new programs as needed;
  • Participate in performance improvement activities, staff trainings, weekly team huddles;
  • Report errors or issues promptly to supervisor;
  • Maintain security of medical records and patient's health information according to HIPAA standards;
  • Order medical supplies as determined by inventory and program needs;
  • Ensure that our patients get the best possible care;
  • Ability to travel to satellite sites and/or offsite locations for support/assignments, events or training as needed;
  • Attend onsite/offsite community engagement activities and onsite/offsite clinic events as needed;
  • Perform other duties as assigned to support HOPE Clinic's Mission, Vision and Values.

Requirements:


QUALIFICATION REQUIREMENTS:


  • Bachelor's degree in Public Health, Biology, Healthcare Administration, or related field are required. Master's degree in above or related field preferred;
  • Minimum of two years in health education, patient navigation, and/or health clinic patient assistance field preferred;
  • Community Health Worker (CHW) and/or Certified Health Education Specialist (CHES) preferred;
  • Knowledgeable of and maintains HIPAA standards of privacy and confidentiality;
  • Microsoft Office;
  • Must be able to utilize the Internet;
  • Office equipment (e.g., computer, copier);
  • Strong written and oral communication skills;
  • Must be detailed oriented and able to handle multi tasks;
  • Detail oriented;
  • Excellent customer service skills;
  • Must be able to handle multiple tasks, complexity, and diversity
