Job Summary and Qualifications:

We are currently seeking a Social Worker MSW to join our team.

As our Social Worker, you will provide discharge planning and assistance with social services to the patient and their family.

You will collaborate with the care team and other healthcare professionals to implement an interdisciplinary process for evaluating a patients' progress from admission through return to the community.

What you will do in this role:

  • You will be responsible for non-clinical, biopsychosocial assessments and interventions as needed for both the inpatient floors and the outpatient emergency department.
  • You will perform crisis intervention, patient/family interventions, high risk screening, focused emotional supports, and referrals for financial assistance or other identified resource needs.
  • You will assist in postacute care placement on all discharges, primarily with those patients identified with postdischarge behavioral health and patient assistance/resource needs, and arranges and facilitates Family Care Conferences to support patient, family, and discharge planning goals.
  • You will be dedicated to patient and family centered care that values personal selfdetermination, prioritization, and engaging in creative, compassionate problemsolving.
  • You will maintain a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes, and possesses a thorough understanding of managedcare concepts and lengthofstay management relating to patient and organizational fiscal responsibility.
  • You will communicate effectively and work with physicians and other members of the interdisciplinary team in meeting patient needs.

What qualifications you will need:


  • Masters of Social Work (MSW) required
  • 1+ years' experience or MSW internship required
  • Licensed Clinical Social Worker preferred
  • Certification in Social Work preferred
  • Critical thinking skills, service excellence and good interpersonal skills.
  • Ability to read and comprehend written/verbal instruction
  • PC Skills
