    Summary



    This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.



    Duties


    The Licensed Practical Nurse (LPN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. The LPN provides nursing care to chronically ill and/or stabilized acutely ill patients who require rehabilitation and nursing services in the CLC, a VA Long Term Care (LTC) facility. The LPN participates in patient care planning and serves as a member of the nursing team.

    Assigned duties include but are not limited to the following:

    • Prepares, administers, records, and communicates results of medications, vital signs, treatments, etc.
    • Reports and records gross and subtle changes in patient's status.
    • Functions as the primary nurse and gives the change of shift report to the oncoming shift nurse and NA
    • Recognizes and initiates appropriate response in emergent situations such as fire, disaster, bomb and/or hostage threats, cardiac and/or pulmonary arrests, and behavioral emergencies.
    • Carries out emergency measures and initiating procedures for obtaining medical/other types of assistance.
    • Initiates CPR as needed.
    • Completes pain evaluations, and on the basis of pre-formulated guidelines, either provides education, or refers for further evaluation.
    • Adheres to asepsis and universal precautions requirements to provide a safe environment for patients, families, and employees.
    • Sets up and cares for patients in isolation per infection control standards.
    • Reviews the patient's progress toward established goals and motivates the patient toward the achievement of these goals.
    • Oversees the Nursing Assistant daily tasks.
    • Manages workload effectively.
    • Documents in medical record patient information including responses to treatments and procedures.
    • Performs military sexual trauma screening as required by VA and facility guidelines.
    • Obtain and process blood, urine and other samples for laboratory testing as ordered by a provider and in accordance with unit and facility guidelines.
    • Establishes and maintains an effective working relationship with peers, coworkers, supervisor, and other health care system personnel.
    • Fosters Teamwork and Communication.
    • Serves as a member of the nursing team and as a member of the interdisciplinary team.
    • Shares observations of the patient's progress/condition, asks appropriate questions, and acts as a patient advocate.
    • Acts as a role model to peers and offers assistance to others.
    • Serves as a member of the Unit Based Shared Governance.
    • Follows health care system directives and Patient Care Services policies, and unit specific policies and procedures.
    • Promotes a safe and therapeutic work environment.
    • Use of Transfer Lifts for Mobility for patients, safely. Use of lift/transfer equipment and good body mechanics per safe patient handling.
    • Maintains ICARE core values and the Mission Statement of the VASNHCS.
    • Supports and adheres to health care system directives, Patient Care Services policies and procedures, and unit specific policies and procedures.
    • Participates in patient teaching activities by reinforcing and reiterating instructions previously presented by the RN, Nurse Practitioner, and/or the physician that provides the patient/family with information for self-care and follow-up home care and outpatient management.
    • Participates in educational in-services.
    • Assists in the orientation of new staff assigned to the unit.
    Work Schedule: Varies, must accept all shifts.
