- Works collaboratively with healthcare providers to facilitate access to high-quality, cost-effective patient care by reducing the time from diagnosis to treatment, improving patient satisfaction and resource coordination.
- Identifies potential patients using radiology reports and Physician appointment schedules initiating contact with a potential patient only after a Physician request or approval.
- Makes patient contact and confirms patient awareness/understanding of diagnosis and the treatment plan including tests, procedures and infusion therapy.
- Collaboration and professional communication with the outpatient neurology team to provide seamless care to the patient during the episode of care.
- Educates and collaborates with neurology staff, physicians, and the community as well as patients and their caregivers regarding services.
- Functions as an expert clinician triaging and responding to patient incoming calls related to infusion therapy, documenting calls and ensuring appropriate notification of MD/APP teams, pending orders for supportive care medications for MD/APP review, monitoring and screening for dose limiting side effects that require immediate intervention.
- Assists in tracking data, outcomes, and supporting strategic and performance improvement activities.
- Guides patients through the health care system by assisting with scheduling appointments, facilitating interaction and communication with healthcare staff and providers, and by making appropriate recommendations for patients to meet their needs.
- Acts as a liaison between patients, Physicians, and infusion team to support their care plan.
- Validates patient concerns and ensures clinical team is aware of any further educational needs or clarifications throughout each step of the process.
- Identifies and develops relationships with personnel in departments involved in the care of the neurology infusion patients; educates providers and general public about infusion process to promote a better understanding.
- Provides materials and resources to patients; participates in hospital sponsored community education programs, and events.
- Serves as a patient advocate.
- Maintains a current knowledge of self-pay information as well as current insurance requirements for infusion medications and patients' insurance benefits.
- Acts as a facilitator in the practice for the coordination of tasks required to process infusion patients through NGPG. Works closely with Practice Administrator to report and resolve operational problems.
- Will be initial point of contact for any staff/patient complaints or concerns related to infusion care.
- Handles problems, inquiries and complaints. Develops positive working relationship with other departments and staff.

Job Summary
The Neurology Infusion LPN Coordinator will help with coordination of all necessary events and orders related to getting patient from MD visit to infusion suite including but not limited to placement of infusion referral order, ordering MRI's per required sequence of care, enrollment with pharma company to get tracer required for diagnostic testing.
The Neurology Infusion LPN Coordinator is responsible for the coordination of the patient through the continuum of healthcare during the patients time within the NGPG practice, and during the time in which they are under the care of an NGPG Neurologist for an infusion medication for a neurological condition.
Significant implementation includes planning, implementation, coordination, monitor, and evaluation of individualized care options, services, resourcing, education and follow up needs.
In addition, in collaboration with Infusion Therapy, PASC, Radiology Services and healthcare professionals, this position coordinates care and services to neurology patients across the treatment continuum.
Connects patients with financial and educational support resources.In conjunction with physicians and other healthcare professionals, the Neurology Infusion LPN Coordinator is responsible for facilitating interactions between patients and physicians and infusion therapy providing logistical support, expediting referrals, and assisting with financial and insurance issues.
