Responsible for facilitating and providing connections to behavioral health services within a system of care for individuals involved in the justice system. Offers clinical services such as behavioral health screenings, short-term case management, coordination, and collaboration with behavioral health treatment providers, San Diego Probation, and additional services.

Primary Responsibilities:

Perform thorough behavioral health screenings in various settings, including local county jails
Identify and maintain knowledge of specific community resources for individuals in the justice system
Coordinate appropriate care linkage to community behavioral health and related services
Assess safety risks and provide crisis management support
Offer short-term clinical case management to ensure consistent behavioral health and medical services
Review available behavioral health history to coordinate services and ensure suitable care levels
Advocate within the behavioral health system of care for clients in the justice system as required
Engage clients in the screening process and collaborate within a team to develop individualized case plans and provide care linkage based on their needs
Use motivational interviewing techniques to assist clients with behavior change
Participate in team meetings to provide education and consultation on linkage to behavioral health and related services
Provide clinical care consultations with various personnel and service providers
Maintain accurate clinical documentation in electronic health records
Utilize technology systems to support service delivery
Comply with relevant privacy regulations
Perform other duties as assigned

Required Qualifications:

Registered psychiatric nurse or California licensed master's level mental health professional
3+ years of direct clinical care experience in a behavioral health treatment setting focusing on assessment, case management, and crisis intervention. Community behavioral health experience

Preferred Qualification:

Familiarity with the system of care in San Diego

California Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is determined by various factors including local labor markets, education, work experience, certifications, etc. UnitedHealth Group adheres to all applicable wage laws. In addition to salary, benefits include a comprehensive package, incentives, equity stock purchase, and 401k contribution (subject to eligibility requirements).
