Description

This position plans and performs case management for pregnant patients. Coordinates health education programs that help patients maintain healthy lifestyles. The Case Manager acts as a resource to assist patients by connecting them with community resources.

ESSENTIAL FUNCTIONS:
_

Participates in case management intake. Makes initial contact with patients via mail or telephone.
Refers patient to appropriate classes as needed.
Coordinates social service appointments as needed.
Follows up with clients who do not keep appointments and reschedules them.
Participates in case management with providers, OB/Gyn, Nutrition, Social Services, and Home Health as indicated.
Performs financial screening for patients and refers patients to internal eligibility department as needed.
Tracks Six Omnibus Budget Reconciliation Act (SOBRA) and Arizona Health Care Cost Containment System (AHCCCS) applicants to determine date of eligibility.
Maintains regular and predictable attendance.
Performs other duties as required.
