About Us
The Senior and Adult Services Division (SASD) directly works with low-income seniors and individuals across all of Maricopa County. SASD works in partnership with many service providers to provide in-home and community-based care.
SASD expects its supervisors and managers to practice core Leadership Principals that include:

We Value- Case management experience assessing for, monitoring, and coordinating in-home and community-based services for individuals who are aging, adults with disabilities, or other vulnerable populations- Master's degree in social work or related field- Moderate written and verbal bilingual skills in Spanish/English
Job Contributions- Use a person-centered model of care, assesses for, authorizes, and monitors services for individuals who are aging and adults with disabilities- Provide empathetic, comprehensive, and prompt care to vulnerable clients- Utilize a holistic approach to ensure assessments cover the full range of needs presented by a client, including, but not limited to, medical, behavioral health, and socioeconomic needs- Complete required case management documentation within strict timelines based on contractual agreements- Utilize community resources/funding in the most effective, efficient, and responsive manner to benefit the well-being of individuals served- Coordinate client care with community partners such as home-care providers, Adult Protective Services, the Public Fiduciary's Office, Community Paramedicine Programs, hospice providers, and others- Perform other duties as assigned, which are consistent with the position and in compliance with agency policies and procedures
Working Conditions- Work conditions will include working in an Office and Direct Service with Case Managers and Clients in the field Must be able to lift and carry up to 20 pounds.
Selection ProcedureMust pass a pre-employment background and/or fingerprint investigation as required by statute or policy, including drug and alcohol testing requirements for positions designated as safety-sensitive
