Position Summary:


The Per Diem Case Manager is responsible for supporting the dignity and independence of older persons in need of assistance by conducting assessments to ensure needs are being met, care plans and program services are appropriate, and ensuring consumers meet program eligibility requirements.

Following the consumer assessment, the Per Diem Case Manager may assist with making initial referrals and/or recommendations for further follow-up, which will be designated to other members of the Home Care team.

The Per Diem Case Manager should expect to work between 10 and 19 hours per week, as needed. This position offers a flexible schedule, including opportunities to work outside of standard business hours and/or weekends.


Job Responsibilities and Performance Standards:


  • Conduct inperson assessments in consumers' homes to determine program eligibility and ensure that the consumer's plan of care and services are appropriate and meeting the consumer needs. Makes initial referrals for services and/or other agency programs as needed. Does so according to Executive Office of Elder Affairs (EOEA), state, federal and Agency regulations and guidelines.
  • As needed, update the Manager of State Home Care Programs and other members of the team of pending referrals, recommendations, and/or changes to the consumer's status that will require additional followups and/or ongoing monitoring, these will be designated to other members of the Home Care department.
  • May participate in interdisciplinary case conferences as requested by the Manager of State Home Care Programs.
  • Conduct checkin calls with consumers as requested.
  • Maintain documentation for consumer records and completion of required paperwork in an accurate, complete, confidential and timely manner.
  • Comply with HIPAA and other state and Agency confidentiality and documentation requirements.
  • Performs other duties as assigned.

Qualifications:


  • Bachelor's degree required.
  • Previous Aging Services Access Point (ASAP) experiencerequired.
  • Experience working within an interdisciplinary team preferred. Must be able to work independently and collaboratively with diverse populations and as part of a team.
  • Exceptional time management and organizational proficiency. Should possess strong communication skills and the ability to multitask in a fastpaced environment and ensure closedloop communications.
  • Must be knowledgeable and comfortable using Adobe and MS Word, Excel, SharePoint, and other Microsoft 365 platforms. Experience with Wellsky a plus.
  • Must be able to travel to consumers' homes via public transportation or by car.
  • Must meet both CORI/SORI and public health screening requirements.
  • Bilingual a plus.

Physical Environment:


  • Physical surroundings are comfortable with mínimal exposure to injury or hazards.

Social/Psychological Conditions:


  • Occasional stress due to periodic or cyclical workload pressures and deadlines. Some interruptions involved.

Physical Effort:


  • Frequently sits, stands, walks, bends, reaches, and stoops throughout the day.
  • Frequently lifts, pulls, pushes, and carries up to 20 lbs.
Periodic eye strain and light ear strain.
