At our 44-bed hospital, we provide specialized inpatient and outpatient rehabilitative services, as well as Home Health services to our patients, serving the communities of Northern Arizona.
We see patients who often are recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.We are seeking a Home Health Medical Social Worker to join our team of passionate patient caregivers

Qualifications:
Required Skills:
- Master's degree from a school of social work accredited by the Council on Social Work Education required.
- Valid MSW license in the state of Arizona required.
- Three to five (35) year experience preferred.
- CPR certification required.
Additional Qualifications/Skills:
- Knowledge of clinical operations and procedures.
- Effective organizational and time management skills.
- Effective written and verbal communication skills.
- Ability to maintain proper levels of confidentiality.
- Ability to work closely and professionally with others.
- Ability to maintain quality, safety, and/or infection control standards.

Responsibilities:
Provides social work services to patients on an intermittent basis in their place of residence, accordance with physician orders and plan of care.
