Elwyn is currently seeking a
Director of Quality Improvement (QI), California to ensure the delivery of quality services to individuals in IDD residential programs, day programs, and Elwyn Industries in compliance with external and internal regulatory criteria and/or standards through the development, implementation, oversight, analysis, and potential realignment of a comprehensive and continuous quality improvement program as required by regulations and Elwyn corporate quality standards.

Direct all quality improvement personnel in IDD Services and the Records. Develop and provide training in quality improvement procedures and outcome-based measures for all IDD employees.

Design, develop and facilitate quality improvement, utilization and safety for individuals supported in connection with risk management activities, incident management, policies and procedures.

Oversee compliance with the physical site and fire safety of all service locations.

Oversee the implementation of the Human Rights Committee and Self-Advocacy Committee for Elwyn CA under the direction of Elwyn Corporate and in cooperation with operations leadership.

The Director of Quality Improvement is a Mandated Reporter and responsible for ensuring that any suspicion of, or knowledge of, suspected abuse is reported in accordance with the law and program policies (SOC 341).


ESSENTIAL FUNCTIONS:


  • Coordinate, integrate and implement a comprehensive and continuous quality management program, including design, plan, data collection, analysis, and reporting on program quality objectives and Key Performance Indicators for the Elwyn California programs and Safety and Security (as related to IDD services)
  • Coordinate, integrate, implement and oversee comprehensive and continuous quality improvement systems and activities including data collection and analysis of family satisfaction and individual supported satisfaction, including complaints/suggestions/concerns and incident reporting, and key performance indicators
  • Develop and oversee the implementation of the Human Rights Committee, including the outreach and identification of members, development of the annual plan and schedule, oversight of documentation of minutes, and general oversight of the committee
  • Develop and oversee the implementation of the Self-Advocacy Committee, including the search and identification of members, development of the framework of meetings and activities, oversight of documentation of minutes, outreach efforts into the community, and general oversight of the committee
  • Implement Home and Community Based Services philosophy and processes through ongoing training, assessment, evaluation and development of processes that support expectations for services funded through HCBS
  • Coordinate and implement best practice evaluations such as National Core Indicators in CA programs
  • Submit quarterly updates on all quality improvement activities to the Executive Director of Quality Improvement
  • Provide supervision of assigned staff in the areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline and termination
  • Direct the work of the Quality Improvement and Records to ensure collaboration between operations, clinical services, nursing, and quality improvement
  • Develop policies and procedures related to quality improvement and program objectives
  • Design, coordinate, and provide inservice training to support compliance, program development and thorough understanding of all applicable regulations for employees in all of IDD Programs
  • Develop and deliver quality improvement focused training for all directors, Administrators and other program personnel to ensure consistency in the quality of delivery of program services
  • Monitor program compliance with quality improvement standards, including but not limited to, the Lanterman Act, Title 17, Title 22, Life Safety Regulations, and Home and Community Services as well as other applicable regulation
