    Description

    Primary Responsibilities

    The LVN Charge Nurse plays a crucial role in ensuring top-notch care standards that enhance the overall well-being of each resident. This involves closely monitoring resident conditions, adhering to care plans, administering medications, and collaborating with physicians when needed.

    Essential Functions
    • Conduct regular rounds to observe, assess, and document resident conditions
    • Ensure proper implementation of care plans
    • Administer medications as per prescriptions
    • Coordinate and document physician visits
    • Manage narcotic reconciliation during shift changes
    • Detect and address significant changes in resident conditions following facility protocols
    • Supervise and support aides
    • Handle Incident and Accident Reports as per guidelines
    • Execute resident discharges based on facility procedures
    • Complete Nursing Assessment for Physician Calls before updating physicians on any changes
    • Communicate with residents' point of contact regarding updates or condition changes
    • May be assigned additional projects by the Director of Nurses or the Administrator
    Qualifications

    Educational/Training Requirements
    • Completion of an LVN program from an accredited vocational school or college
    Licensing Requirements
    • Active Licensed Vocational Nurse status in Texas
    Experience Requirements
    • Preference for experience in a skilled nursing facility
