Banyan Treatment Centers is recruiting a full-time Case Manager to join our treatment team in Milford, DE.


The
Case Manager is responsible for performing a wide variety of client care activities as directed by the Executive and Clinical Director.

The Case Manager collaborates with the Treatment Team to formulate the Master Treatment Plan, assists in coordinating discharge planning, provides support to clients and Clinical Team, and could have the responsibility for maintaining communication with the referral source and other services provided as needed.

Essential Functions:


  • Contact referral source and other key individuals to gather additional information as necessary.
  • During first session with patient, review initial treatment plan, including case management issues and be sure that patient and staff assignments and responsibilities are understood and in progress.
  • Formulate a comprehensive treatment plan as it pertains to continuing care and is completed within established time frames.
  • Develop clearly stated treatment goals and objectives that are measurable.
  • Develop interventions that will accomplish goals and objectives and state the planned frequency of interventions as it pertains to continuing care.
  • Document the clients progress accurately as it relates to case management.
  • Continue treatment planning process throughout clients stay in treatment, as it pertains to medical, legal, and employment issues, opening new problems and developing or revising treatment plans as necessary to assist in continuing care.
  • Provides case management to patients at least one (1) time per week or more frequently if needed.
  • Assist clients in understand treatment goals as it pertains to continuing care plan.
  • Maintain regular contact and open communication with families.
  • Assist families in finding resources to help themselves.
  • Link patient with internal and external resources as needed to meet patient needs.
  • Maintain contact with referral source and other key individuals.
  • Assist patient in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the patient's progress in recovery.
  • Prepare patient to maintain recovery postdischarge.
  • Develop a discharge plan that meets patient needs for ongoing support.
  • Assist in clinical interventions and staffing of patients if needed.
  • Administer the Coolidge Assessment Battery Pre and Post assessment.
  • Will work with Clinical Case Manager Team Leader to maintain compliance and patient care.

Preferences:


  • CAP.
  • Bachelor's Degree in a Human Services related field.
  • Work experience as a Case Manager in the field of substance abuse or mental health.

Qualifications:


  • Associate's degree and/or 1 to 3 years of work experience in the field of substance abuse or mental health
