Job Title: Care Coordinator
Job Summary:
The Care Coordinator is responsible for planning, facilitating, and documenting collaborative care efforts with public and private service providers. This includes coordinating referrals for temporary housing, quick rehousing, and permanent supportive housing while ensuring accurate data collection and reporting on client progress.
Minimum Qualifications:
To excel in this role, you'll need an Associate degree, relevant experience in health or human services, and knowledge of recovery support services. Experience with substance use disorders, mental health diagnosis, and chronic conditions is preferred.
