The services are designed to build independent living andtenancy skills and connect people to a full range of services, including case management, recovery treatment, health care, vocational and employment support, access to benefits, money management, daily living assistance, and social and recreational activities.
Position DescriptionPosition Summary:
The Supportive Housing Coordinator will provide engagement and supportive services to empower residents and to promote stability and recovery through coordination of services, education, and referrals to community resources.
The Supportive Housing Coordinator will work closely with Housing Facilitation, Property Management (including Maintenance Technicians), support agencies, and Service Providers to address the service needs of tenants so that they may achieve and maintain housing stability.
The Supportive Housing Coordinator maintains an approximate 1:50 ratio for independent housing units in their assigned region and acts as the main point of contact for these units.
This ratio will change based on the portfolio needs.Responsibilities:
Service Coordination and Treatment PlanningDetermine service needs of tenants; develop and regularly monitor implementation of an Individualized Housing Service plan; Ensure plan includes appropriate service providers and community resources.
Ensure housing retention by supporting residents to maintain tenancy obligations (e.g., timely rent payments, lease requirements) and required documentation for subsidized housing.
Link resident with services and collaborate with resident's ongoing treatment providers to assist with housing retention.Assist consumers with housing-related issues such as household setup, budgeting, housekeeping, contacting property management for repairs, health, and safety procedures.
Collaborate with property managers around housing retention including:
payment plan and housing retention plan development, housekeeping inspection walk through, and resolution planning.
Provide creative engagement at a minimum weekly for the first 2 months following housing and then once monthly as determined by resident's needs.
Assist tenants with achieving greater economic security by linking them with programs to improve job readiness or assist with application for benefits.
Crisis InterventionProvide and coordinate support as needed when resident's experience crisis; Utilize harm reduction, motivational interviewing and crisis intervention techniques to maximize client success.
Monitor tenant wellness through regular needs assessment and lethality screening. Develop safety plan with tenant as needed to ensure harm reduction. Review assessment with Supportive Services Program Manager within 24 hours.Outcome Measurement and DocumentationProvide accurate, timely documentation to ensure program compliance with funder requirements. This includes collecting programmatic demographic and outcome data, conduct periodic service utilization reports, and grant reports as needed.
Participate in regular case review and monitoring to ensure compliance and accurate record keeping.
Supportive Mediation and AdvocacyCoordinate and facilitate support team meetings for each resident; Attend all scheduled partner meetings as requested.
Work in collaboration with property managers to address tenant needs; advocating and ensuring tenant perspectives are included.
Work in collaboration with ADAMH and Housing Facilitation to assist MH Case Manager and applicant to identify third party landlords and submit units to CMHA for inspection when housing a client in the Mainstream Voucher program.
Other:
Provide knowledge and guidance to support organizational initiatives related to profession; Lead, guide, and train employees, interns, and volunteers performing related work.
Required Skills, Experience and Qualifications:
Bachelor's Degree in Social Work, Psychology, Human Services, or related field and a minimum of 2 years' experience in social services work, preferably in housing or community mental health OR in lieu of a Bachelor's Degree, a minimum of 4 years' experience in social services work, preferably in housing or community mental health setting.
Proficient in Microsoft Office Suite Products and ability to learn and adapt to changing technology utilized in the role.
Understanding of the Housing First model and Trauma-Informed Care required.
Proficient demonstration of Harm reduction, self-determination, and unconditional positive regard philosophies Strong verbal communication skills required; ability to communicate across all levels both internally and externally.
