Overview:
We are seeking a compassionate and skilled Hospice Nurse to join our team.

As a Hospice Nurse, you will provide specialized care to patients with life-limiting illnesses and support their families during this difficult time.

Duties:

Provide comprehensive nursing care to hospice patients in accordance with their individualized care plans
Administer medications, monitor vital signs, and assess patient condition
Collaborate with the interdisciplinary team to develop and implement patient care plans
Assist with pain management and symptom control
Educate patients and families on disease progression, treatment options, and endoflife care
Coordinate with other healthcare providers to ensure continuity of care
Conduct regular assessments and document patient progress
Assist with discharge planning and facilitate transitions to other levels of care as needed

Qualifications:

Valid nursing license (RN or LPN)
Experience in hospice care is preferred but not required
Familiarity with discharge planning systems and processes
Proficiency in medical terminology, ICD9 coding, and electronic health records (eClinicalWorks)
Strong communication and interpersonal skills
Ability to provide compassionate care in a sensitive and empathetic manner
Knowledge of aseptic technique for specimen collection processing
Reliable Transportation. Valid Auto Liability Insurance

Rate of Pay is negotiable upon discussion with Administrator.

Pay:
$ $40.00 per hour
