JOB DESCRIPTION:
Summary:
AZCFC has an 18 year proven track record of providing evidence based behavioral health services to the community. We are seeking a highly competed professional for Intake Coordinator. This is a unique and exciting opportunity for an ambitious, self directed individual.
In this position, you will have the opportunity to coordinate all aspects of the agency's intake and case assignment process.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Coordinates all activities associated intake procedures, assessment and treatment assignments of agency clients.
- Develops a system of tracking agency client statuses from Intake to engaging in treatment. Develops and implements a system of follow up for clients who do not engage.
- Conducts a large majority of agency intake assessments by obtaining appropriate consent forms and completing behavioral health assessments.
- Follows up on all intake and assessments that were conducted by other staff and ensure completeness of the intake and assessments.
- Develops a system of follow up to make sure clients successfully engage in the appropriate programs.
- Provides training to new staff regarding intake and assessment procedures.
- Completes weekly entries and associated duties on the Intake Coordination deliverable per management request.
- Ensures reports from this deliverable are available to review at management meeting per management request.
Skills and Qualifications:
Applicant qualifications required include a minimum of master's degree in mental/behavioral health field and possess a current license through the AZ Board of Behavioral Health Examiners.
- Applicants should have at least 2 years of experience completing comprehensive clinical assessments, diagnosing and identifying clients' clinical needs in the behavioral health field.
- Task and detailed oriented.
- Proficient at developing tracking systems for case follow up.
- Is able to proficiently navigate Microsoft Excel and can create meaning charts, graphs, etc.
- CPR/1st aid.
- Proficient knowledge of DSM-V diagnostic criteria.
- Ability to capture assessment data detail and properly document.
- Ability to plan own work and carry out assignments effectively.
- Knowledge of existing social and behavioral health resources available for client care.
- Must present a current Class I Fingerprint Clearance Card prior to employment.
- Ability to implement crisis skills and risk tools to manage client risk.
- Strong organizational skills.
- Excellent verbal, nonverbal, and written communication skills.
- Ability to participate as an effective and efficient team member.
- Bilingual a plus.
Note
:
Work schedule is Monday-Friday with two nights scheduled until at least 7pm.
Pay:
$ $31.25 per hour
Expected hours:
No more than 40 per week
