- Schedule: 8AM – 5 PM
- In collaboration with provider, performs initial assessment and intake obtaining social, medical, mental health, substance abuse, and medication history
- Provides education and assessment to patients looking to access treatment programs
- Offers referral services for patients looking to access treatment programs
- Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment
- Provides ongoing management, education, and support of patients in all phases of treatment including; nursing visits, urine toxicology screens, routine labs, medication teaching, monitoring, pill counts, observed dosing, and medication refills
- Follow state and federal guidelines in providing care to opioid-dependent patients in collaboration with license, prescribing providers, and pharmacy
- Complete appropriate documentation for all patient visits and phone calls in a timely manner, ensuring that all notes are complete and easily understood
- May be required to float to the South Sioux City location once MAT services are offered at that location
- These functions are not all inclusive
- Minimum of RN license, BSN preferred
- Current BLS and Mandatory Reporter certifications
- Clinical office and case management experience preferred
- Experienced RN with a background in case management who is self-motivated and capable of working with others to improve patient care
- Starting salary: $52,000 - $76,000 annually with differential if bilingual.
