Job Summary and Qualifications:
What you will do in this role:
- You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.
- You will perform onsite and offsite clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
- You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
- You will obtain information relating to patient resources and healthcare insurance benefits.
- You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.
What qualifications you will need:
- Basic Cardiac Life Support must be obtained within 90 days of employment start date
- Speech Therapist, or Licensed Physical Therapist, or Registered Nurse, or Occupational Therapy (OT)
- Minimum of two years clinical experience
- Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 lbs.
- Case Managment experience preferred
