Position Overview:


  • Under the direct supervision of the Outreach Team Lead, the Community Health Worker is responsible for supporting Harris County Public Health's outreach activities and serves as the key liaison to educate, promote, and connect community members to all HCPH programs/services specifically on behalf of the Maternal and Child Health and Clinical Health Services Programs. This position will primarily work in the community through various outreach activities within Harris County, while also completing inoffice tasks/projects to support overall outreach team objectives.

Duties and Responsibilities:


  • Responsible for diverse outreach, education and community coordination activities such as canvassing local businesses/organizations, attending community events, conducting outreach phone calls, and giving presentations
  • Maintain knowledge/skills to educate, promote, and connect community members to all HCPH programs/services
  • Track, collect, and submit outreach service data on a daily and weekly basis
  • Build and maintain relationships with stakeholders and organizations to create and sustain innovative partnerships
  • Responsible for initiating referrals to HCPH programs/services, while ensuring community members' confidentiality
  • Create and deliver educational presentations/workshops on public health topics for diverse target audiences
  • Other duties as assigned, including special tasks involved in responding to an emergency event.

Requirements:
Education:


  • High School diploma or GED equivalency
    Knowledge, Skills and Abilities:
  • Intermediate proficiency in Microsoft Office-
  • Must communicate clearly and effectively, both orally and in writing
  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
  • Ability to maintain confidentiality when dealing with sensitive information

  • Monday

  • Friday | 8:00 A.M. 5:00 P.M.
  • Other days and hours as necessary

Work Environment:


  • This job operates in a professional office environment.
  • This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

Physical Demands:


  • This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
