Program Director or Program Manager


JOB SUMMARY:


The Program Supervisor is responsible for assisting the Program Director/Manager in the day-to-day supervisory and administrative management of the residential program as well as providing a variety of case management and housing related services to clients to facilitate their adjustment to and integration into the community.

He/she is responsible for completing tasks including but not limited to:
supervising case management staff to ensure compliance with charting/documentation, preparing monthly statistical reports (e.g. food stamps, food expense, and petty cash), handling client benefits and following up with SSI, Food Stamps, Public Assistance, etc.

Program Supervisor is also responsible for conducting/reviewing fire drills, ensuring compliance with safety checklists, as well as ensuring that a clean, safe environment is maintained for all residents.

Program Supervisor must monitor and ensure medication compliance, attends and actively participates in monthly and annual trainings/in-services both on and off-sites, and overall represents the department and hospital in a professional manner.


KEY RESPONSIBILITIES;

  • Supervises case management staff (e.g. provides direct supervision to case managers including review of client records to ensure compliance with regulatory agencies; case consultation, etc.) Ensures staff is completing charting paperwork/documentation in a timely manner.
  • Participates in the planning and developing of programmatic elements of the residential program.
  • Coordinates/prepares staff schedules to ensure coverage for all shifts if instructed.
  • Prepares monthly statistical reports, levels of services, financial and other reports as required by the NYS Office of Mental Health, NYC Department of Mental Health and other regulatory agencies and/or as required by and in accordance with the standards set forth by St. Vincent's Hospital Westchester (i.e. Food Stamps, Food Expense, Petty Cash).
  • Assesses client needs and integrates case management services into total care of clients.
  • Participates in coordinating case management services with client and involved collateral agencies. Provides referral and linkage where needed.
  • Provides evening night and weekend coverage necessary for unit coverage.
  • Provides concrete and supportive services to clients and their families related to psychiatric aftercare, crisis intervention, benefits, socialization, recreation, vocational, educational services, counseling as well as independent living skills training.
  • Formulates appropriate service plans including completing comprehensive written service plan within 10 days of admission and quarterly reviews as specified by Residential Services policy and procedures when/if assigned a caseload.
  • Advocates on behalf of clients in resolving issues and mediates problems related to provision of appropriate services including maintaining regular contact with involved agencies and providing in-person outreach when necessary.
  • Assesses clients independent living skills training and/or restorative services training including assisting clients in cooking, cleaning, shopping, menu planning, personal hygiene, budgeting, leisure activities, etc.
  • Monitors client's benefits and provides budgetary counseling, if necessary (e.g. Medicaid, Social Security, Public Assistance, etc.)
  • Enters maintenance requests as needed and ensures that they are being completed in a timely manner. Follows up with Property Management when/if necessary.
  • Ensures that residence/apartment meets all safety/fire regulations and follows up with vendors as necessary.
  • Conducts/reviews fire drills and self-preservation tests for clients.
  • Assists residents in maintaining a clean, safe, homelike environment.
  • Monitors and ensures medication compliance, including monitoring client's prescriptions through counting of medication (pills), providing supervision of medication if necessary and providing medication skills training.
  • Maintains valid Driver's License and provides transportation via residential car or van to residents when required.
  • Attends weekly supervisory sessions and shares information during staff meetings and rounds.
  • Represents the department and hospital at appropriate meetings and conferences within the hospital and community.
  • Adheres to standards for timely submission of notations and documentation.
  • Ensures the quality of progress notes and related clinical documentation concerning client care. Adheres to professional writing standards (grammar, spelling and composition).
  • Attends and actively participates in monthly and annual trainings/in-services both on and off-sites.
  • Completes other assignments given by the Program Director, Program Manager and/or Management team.
  • Adheres to SJMC and departmental policies and procedures
