Through the Office for People with Developmental Disabilities (OPWDD), Self-Direction is a waiver service that enables participants to select services, self-hired staff, and schedules that work best for them.

Self-Hired Community Habilitation is an approved service offered through OPWDD's Home and Community Based Services (HCBS) Waiver Program.

Community Habilitation is a goal-oriented service that aids in developing and improving skills that will help a person with determination increase their functioning.

Ultimately enabling persons with Developmental Disabilities to gain the skills necessary to be an active participant in their homes and communities.

Community Habilitation Staff assist individuals in increasing self-sufficiency by providing the following services and supports:

  • Activities of Daily Living (personal care, meal preparation, household chores)
  • Communication Skills
  • Socialization Skills
  • Travel Training
  • Money Management
  • Community Inclusion
  • Expanding the Individual's knowledge and use of Community Resources
Community Habilitation services can be provided in a person's home, as well as within their surrounding communities. The times and schedules of the Self-Hired Com Hab are arranged individually by the client and/or the parent/advocate.

Community Habilitation is not a babysitting, home care or respite service.

Community Habilitation is intended to work on goals that are mutually agreed upon by the client, the parent/advocate and service provider.


Case specifics:


Qualifications

  • 18 years of age or older
  • High School Diploma or equivalent required plus one year experience working with Developmentally/Intellectually Disabled individuals preferred
  • Meet all criteria to be considered a valid driver for the agency
  • Ability to be flexible and work in a team environment
  • Excellent interpersonal, oral and written communication, decisionmaking, and problemsolving

Flexible Start and End Time (Day Time Hours)

10hrs max per week
