- Assist in identifying and reaching out to medically complex patients.
- Collaborate with the multidisciplinary team to develop and execute care plans.
- Engage with patients through home and hospital visits, and accompany them to provider appointments as required.
- Conduct Vital Signs assessments.
- Connect members with necessary resources by identifying care gaps.
- Coordinate telemedicine visits between members and providers.
- Provide disease-specific education.
- Perform safety evaluations in patients' homes.
- Communicate any concerns to the clinical team and manager, seeking proactive support and guidance.
- Reinforce established treatment plans.
- Establish relationships within the community to access available resources for patients.
- Hours: 8 am to 4:30 pm, with one later day per week from 9:30 am to 6 pm (No weekends or holidays).
- Travel: Yes, this is a community-based role covering Lackawanna and Luzerne Counties.
