Job DescriptionWe are seeking an mental health Care Coordinator to join our team You will be providing clinical and administrative support to patients receiving psychiatric care at our outpatient mental health clinic.
Essential Duties And Responsibilities:
Sending medication refill approvals to pharmacy upon approval by the medical provider.
Obtain Prior Authorizations from insurance companies ..
Sending lab orders and communicating with patients about results in a timely manner
Other duties as needed including follow up appointment calls, assisting with requests, assist with letters, forms, and other documentation requests by patients for school, employment, disability etc.
Maintain case files within electronic health record (EHR) that are up to date, including lab results, medication adjustments, coordination of care, etc. Maintain confidentiality within the workplace and within the community.
Facilitate referrals to other healthcare professionals and medical facilities.
Required Knowledge, Skills, and Abilities: -
Some knowledge of medications and medication management
Experience in social service or mental health field preferred
Skill in working independently and time management
Understands patient confidentiality according to HIPAA standards
Has basic computer skills, including Word and Excel and be proficient with a multi-line phone.
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment
