Americare Nursing is hiring PAS Field supervisor/Community Liason.
Requirements:

MOST HAVE EXPERIENCE MINIMUM 1-2 YEARS

Assess client's functional status and take necessary action to ensure client's well-being
:

Promote, review, and evaluate the quality and appropriateness of client care practices
:

Ensure services are to be delivered in full compliance with the DADS/HMO's regulations, contract standards, and agency policy procedures
:
-
Responsible in the recruitment of new members/clients, interview, hire, train supervise, evaluate and terminate (if needed) attendants needed, provide services to clients, serving as an advocate for members of the community:
-
Conducting surveys in neighborhoods to determine the needs of the community and referring them to the appropriate government organizations:
-
Will also be responsible for conducting presentation in the community informing its member of services that are provided
:

Must also act as a consultant to client regarding interpretation of program related federal, state and agency regulations and policies
:
-
Maintain current/accurate field charts or client master list that may be used for scheduling, hiring and supervisory visits
:
-
Make initial visits to each client's home with the attendant on/or before initiation of service for the purpose of developing a plan of care, orientation and hiring the attendant
:
-
Complete all paperwork belonging in client charts and copy to clients
:
-
Review and act to resolve complaints made by client, personnel and service management needs and problems
:

Participate in assigning attendants to client based on client needs
:
-
Project a concerned, professional attitude and develop a positive rapport with community resources, caregivers and all staff
:

Assist if needed in inputting new clients into Vesta to include scheduling/schedule changes of attendants and proper documentation to include all new/updated authorizations/3052'S to be incompliance with HHSC policies, initiate changes in the system and print out necessary paperwork and complete appropriately and submit to corporate office for filing in original charts

Responsible for having new and yearly forms/packets for appropriate visits (prepare your packets for
SOC's, yearly's and 6 months visits.

Responsible for scheduling all visits into vesta and update yearly visits to include printing out monthly SV/Annual list for completion of visits and completing all paperwork with changes pertaining to increases/decrease of units/tasks/change of schedules etc

Assist with any other assignments as assigned by PAS Program Supervisor.

$20.00 per hour
Expected hours: 40 per week
