As a Medication Reconciliation Specialist, you will play a vital role in ensuring the accuracy and safety of our clients' medication regimens.


Key Responsibilities:


  • Conduct interviews with patients to obtain comprehensive medication histories.
  • Communicate with pharmacies and physician offices to verify and update medication information.
  • Assist pharmacists by serving as a knowledgeable resource in medication reconciliation processes.
  • Support nursing and therapy teams in accurately transcribing medication orders.
  • Collaborate with healthcare professionals to resolve discrepancies and ensure seamless transitions of care.

Requirements:


  • Active Registered Nurse (RN) license.
  • Strong attention to detail and organizational skills.
