The
Community Health Worker will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.


Working Schedule:
Schedule is Monday through Friday standard business work hours 8am to 5pm Central Standard Time.


If you are located within Harris County, Northeast, Central, TX, Tarrant and Bexar County, TX region, you will have the flexibility to telecommute* as you take on some tough challenges.

This role requires some field-based work and home-based office.


Primary Responsibilities:


  • Assess, plan, and implement care strategies that are individualized by the member and directed toward the most appropriate, lease restrictive level of care
  • Utilize both company and communitybased resources to establish a safe and effective case management plan for members
  • Collaborate with patient, family, and healthcare providers
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate with all stakeholders the required healthrelated information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team
  • Document all member assessments and referrals provided
  • Participate in Interdisciplinary team meetings
  • Understand insurance products, benefits, coverage limitations, insurance, and governmental regulations as it applies to the health plan
  • Understand role and how it affects utilization management benchmarks and quality outcomes
  • You'll need to be flexible, adaptable and, above all, patient in all types of situations
  • Engage members either face to face or telephonically
  • Adhere to detailed, specific documentation requirements in the member's health record
  • Proactively engage the member to manage their own health and healthcare using Motivational Interviewing Skills
  • Utilize solid skill sets of managing multiple tasks at a time, being selfmotivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and ability to manage multiple deadlines

Required Qualifications:


  • High School Diploma/GED (or higher)
  • 3+ years of experience working within the community health setting in a health care role
  • Familiarity with Microsoft Office, including Word, Excel, and Outlook
  • If required, reliable transportation and the ability to travel in this 'assigned area' to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices if field based
  • Ability to travel for quarterly meetings
  • High speed internet in residence

Preferred Qualifications:


  • Bachelor's Degree in Social Work, Licensed Practical Nurse (LPN), or CNA/HHA
  • Reside within commutable distance of assigned duties
