    Job Summary:

This is an intensely collaborative role with the Regional Director of Clinical Operations (RDCO). Communication is a significant aspect of this role; when in doubt over-communication is better than lack thereof. This role will continue to evolve as the company grows. The regional lead clinician was developed out of a need for each clinician to have a go-to person for liaising between senior-level management and the clinicians/facilities.

    • Attend pre-implementation meetings within the region
    • Serve as a support person during implementations and re-implementations; recommend being present for the first 2-3 weeks of new facility implementations

    • Assist with skin sweeps/ensure skin sweeps are done quarterly
    • Assist in coordinating rounding schedule with Providers and Facilities (minimum twice weekly visits in every facility)
    • Identify key opportunities for improvement and increased efficiency
    • Utilize a centralized scheduling tool (coming soon) to make available providers' schedules to the senior management team; this is another area where more communication is better than less, especially when facility management/ownership personnel reach out directly to our CEO & COO
    • Measure and report performance of providers in a regional area in the following key functional areas:
      • Quality patient care and clinical effectiveness
      • Effectiveness and efficiency of clinical rounds, following appropriate care models, processes, and standardized procedures of LTC Healing Partners
      • Quality, accuracy, and timeliness of documentation

    • Perform regular (no less than weekly) check-ins with clinicians
    • Perform regular check-ins (no less than monthly) with facilities and regional nursing leadership to ensure strong and positive relationships are established and maintained on behalf of LTC Healing Partners
    • Assist RDCO with planning & coordination for new facility contracts and provider placementStrategic employment of providers throughout the region should be discussed with senior management before significant changes are made due to preexisting agreements and client requests
    • Ensure appropriate coverage at all facilities under regional purview - identify and report shortfalls in staffing and performance to senior management and potential corrective measures as proactively as possible
