_Seeking a full-time organized, detailed oriented, pragmatic, and collaborative community health worker to join the community health team based in our Lake County office.

_


General Responsibilities

  • Through home visits and in clinic and community settings, works 1 on 1 with clients to secure information such as medical, psychological, and social factors contributing to client's situation.
  • Records information in database, works to identify and navigate resources to support the client and eliminate barriers to good health.
  • Advocates for clients and acts as a liaison between clients and other service providers.
  • Helps clients and families understand, accept and follow medical recommendations.
  • Provides healthcare enrollment services, Medicaid Outreach, and DHD#10 services outreach in community settings.
  • Covers primarily Lake County with portions of eastern Mason County and southeast Manistee County.

Minimum Requirements

  • High school diploma or equivalent required.
  • Strong people & counseling skills.
  • Certified Community Health Worker required or ability to acquire.
  • Ability to effectively interview clients in clinic, home, and community settings, present information 1 on 1 &/or small groups to clients & community organizations.
  • Ability to work with a team & clinic environment.
  • Ability to utilize computer electronic medical record.
  • Reliable transportation & occasional nontraditional hours.
  • Experience in social work &/or public health preferred.

Schedule:

Education:


  • High school or equivalent (preferred)

Experience:

  • Public health: 2 years (preferred)

License/Certification:

  • Driver's License (preferred)

Ability to Relocate:

  • Baldwin, MI 49304: Relocate before starting work (required)
