- Demonstrates both independence and excellence in conducting thorough health evaluations for the Medicare Advantage patient group.
- Works in conjunction with the primary care physician to devise a care strategy centered on prevention, early identification, and prompt intervention.
- Adheres to the permitted medical practices in the state of licensure and employment.
- Maintains open communication with physicians and family members during care transitions.
- Efficiently communicates with all team members and facilitates a suitable and efficient care strategy to prevent unnecessary duplicity of tests and fragmentation of care.
- Completion of certification through either the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) with a focus on adult medicine.
- Currently licensed as an Acute, Adult/Gerontological, or Family Nurse Practitioner in the practicing state or eligible for licensure before employment.
- Has or is eligible for DEA and, if necessary, CDS licensure in the state of practice.
- Current CPR Certification and Basic Cardiac Life Support certification.
- Strong organizational and prioritization abilities.
- Proficient in handling Electronic Medical Record (EMR) systems, particularly ECW.
- Familiarity with Medical Advantage, HEIDS, MRA, and ICD-10 Coding.
- Comfortable with the management of predominantly Medicare-aged patients.
- Demonstrated capacity to solve intricate and diversified situations.
