Description

As the Community Partner Coordinator, you serve as a critical link for our patients and New Bedford Community Health (the Health Center) to assistance offered by community agencies and hospital-based services.

By collaborating with the multi-disciplinary teams at the Health Center, you will help patients navigate services that are available to them.


The CP Coordinator is also a vital advocate for our patients within the Community Care Cooperative (C3) Accountable Care Organization (ACO) that the Health Center is affiliated with.

Managing your patients' Person-Centered Treatment Plans (PCTP) from the ACO, you will ensure that your assigned patients have the appointments they need and any other specialty services required in their plan.

  • Participates in the overall optimal functioning of the health center.
  • Maintains and encourages a teamwork approach
  • Maintains consistent effort to further goals by modifying and or improving individual procedures and tasks.
  • Enjoys and embraces working in high volumes and diversified frequencies and activities.

  • Monday Friday: Work Schedule (8:00am

Qualifications:


What you will need:


  • Experience working in the social, behavioral, and medical health systems is preferred.
  • Excellent relationship building, communication and engagement skills.
  • Strong organizational skills and attention to detail.
  • Good Computer skills including proficiency with Microsoft (Office, Excell, and Word).
  • Ability to positively influence others with respect and compassion.
  • Ability to provide excellent customer service to community agencies and patients.
  • Ability to function effectively as a member of a multidisciplinary Team.
  • CPR Certified
