- Provide comprehensive mental health care and support, including the diagnosis and treatment of mental health disorders.
- Develop tailored treatment plans based on individual patient needs and circumstances.
- Regularly meet with patients for counseling and treatment, adjusting plans as needed.
- Collaborate within an interdisciplinary team, participating in treatment team meetings to discuss clinical cases and update patient treatment plans.
- Engage in continuous monitoring and coordination of behavioral health services, linking with other treatment providers both within and outside the primary care setting as permitted.
- Facilitate patient and family education and self-management skills, ensuring full participation in their treatment.
- Maintain detailed treatment and progress reports, documenting in patient medical charts.
- Other duties may be assigned.
- Ability to work effectively in a fast-paced primary care environment.
- Excellent interpersonal skills for a multidisciplinary team collaboration.
- Proficient in brief diagnostic assessments for mental health and co-occurring disorders.
- Strong organizational skills and proficiency in Microsoft Office Suite and electronic medical records.
- Successful completion of background check and drug screen.
- Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures.
- Pennsylvania Licensed Clinical Social Worker (LCSW)
- Master's degree in Psychology, Social Work, Counseling, or a related clinical discipline from an accredited college or university
- Knowledge of HIPAA compliance and ability to handle confidential information.
- At least 3 years of experience in the field as an LCSW.
