Job Summary:
The Outpatient Integrated Care Manager is responsible for the overall operations and management of the outpatient care clinic.

This position is responsible for overseeing daily operations and workflow within the clinic to ensure standards of practice performance are met.

This position will provide management, training and technical assistance to the appropriate staff within the Integrated Care Clinics.

It is Responsible for assisting in identifying and correcting gaps in care, implement care management protocols-criteria, and screening tools for health related social needs.

Assists in developing and implementing protocols for high risk clients, promoting client health activities, AHCCCS Targeted Investment Program (TIP) including identifying, measuring and reporting outcomes.


Supervisory Responsibilities:


  • Recruits, interviews, hires, and trains employees.
  • Supporting growth and development of managers and clinical coordinators.
  • Provides constructive and timely performance evaluations.
  • Recommends compensation adjustments.
  • Handles discipline and termination of employees in accordance with company policy.
  • Involved with handling all employee relation issues.
  • Timecard responsibilities.
  • PTO management.

Essential Functions:


  • Manages daily operations of the Integrated Care Clinic.
  • Provides operational supervision to appropriate clinic staff.
  • Responsible for ongoing collaboration with the Integrated Care Director and other management personnel to assist that a smooth and positive care plan is set in place for each client including monitoring and improving monthly outcomes.
  • Manages the workflow and ensures appropriate continuity of care is being implemented for each client as well as the coordination of care between clinic staff and other departments within the organization.
  • Implementing productive workflows and protocols within the Integrated Care Department.
  • Ensure integrated care team is providing necessary coordination and quality of care for members needing PCP services, psychiatric care and substance abuse treatment.
  • Assist in maximizing the utilization of population health platforms potential for metrics and reporting which include: PRAPARE, Valuebased contract measures, AHCCCS Targeted Investment Program (TIP) health inequities, social determinants of health, and other organizational initiatives.
  • In collaboration with Integrated Care Director will assist and monitor the progress of new initiatives; which include establishing report metrics, methodologies and standards using all data sources to track and monitor HEDIS performance.
  • Through the direction of the Integrated Care Director, drive innovative solutions to enhance quality of care and positive outcomes.
  • Tracking client related incidents to assist in reducing risk to improve client quality of care.
  • Work closely with community resources and organizations in coordinating client services to address gaps in care and utilization.
  • Complete and file all documentation as required in a timely manner.
  • Coordinates with management personnel to ensure each department is collaborating and working together appropriately.
  • Monitor incident reports within the Crossroads company and ensure clients are connected to the appropriate team and that necessary follow up is assigned.
  • Maintain Arizona Department of Health state standards and requirements.
  • Attend and participate in individual and/or peer supervision as directed.
  • Helps to maintain and maximize provider appointment availability to reduce gaps in care and improve quality of care.
  • Perform other duties, as required.

Qualifications:


  • Valid DPS Class One Fingerprint Clearance Card.
  • Current Arizona Driver License.
  • Successfully pass a Background Check.
  • CPR/First Aid training and have a negative TB test.

Knowledge, Skills and Abilities:


  • Demonstrated strong organizational skills.
  • Ability to communicate effectively at multiple levels, both verbal and written.
  • Ability to work independently and as a member of a team.
  • Provide strong leadership qualities as well as problem solving skills.

Education & Experience:


  • Minimum of High School Diploma or GED required.
  • 23 years of behavioral health management experience or equivalent combination of education and experience.

Work Environment:


  • Fast paced behavioral health residential setting.
  • Primarily indoors, however, some outdoor work may be requested.
