THE POSITION:


  • Work for an agency that provides valuable services to people aged 60 and older. This is one of the fastest growing segments of the population in Lebanon County.

DESCRIPTION OF WORK:


  • This Care Manager will be responsible for the Pennsylvania Caregiver Support Program/OPTIONS which links care giving adults with support and reimbursement (financial) to assist in the care of care receiver. You will be receiving case assignments involving general casework and assessment procedures. Work involves interviewing and evaluating the consumer in their homes and developing a plan of care for the consumer. You will have the opportunity to provide counseling and support for consumers and their families to help them adjust to difficult situations and make them aware of the programs that our agency offers and how it may help solve some of their problems.

Work Schedule and Additional Information:


  • Fulltime employment
  • Work hours are 8:00 AM to 4:30 PM, Monday
  • Friday, with an unpaid 30minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY:


QUALIFICATIONS

Minimum Experience and Training Requirements:


  • Six months as an Aging Care Manager 1 or a County Caseworker 1;
    or
  • Successful completion of the County Social Casework Intern program;
    or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;
    or
  • An equivalent combination of experience and training

Other Requirements:
