Description

The Intake Coordinator is responsible for each step of the intake process of our home health referrals, acting as the point of contact for discharge planners, physicians, and agency staff.

They are responsible for sourcing and collecting patient information, distributing information to the appropriate team members, maintaining patient files, setting up charts, and coordinating with clinicians to verify that appropriate services are provided in a timely manner.

They will also have general administrative duties such as answering phones and filing/faxing paperwork as needed.


Hours of Work:
Full-time, Monday-Friday 8:00AM-4:30PM. This position will be daytime hours only (no nights, weekends, holidays).


Duties:

  • Initiate and facilitate the referral process for all patients.
  • Manages patient information within the EMR
  • Answers phone calls and responds to inquiries related to our agency.
  • Assists clients in their homes with personal cares including bathing, dressing, grooming, skin care, oral cares, shaving, assistance with ambulation, toileting, and showering/bathing. Other duties as assigned in client's Plan of Care.
  • Assist with household duties which may include light housekeeping, laundry, changing linens on beds, mopping/sweeping/vacuuming floors, cleaning bathrooms, meal prep, organization, etc. Other duties as assigned in client's Plan of Care.
  • Assist clients with selfadministration of medications.
  • Engage in respectful, meaningful social interaction with clients, including friendly conversation and empathic support, as well as respecting client's privacy and property.
  • Maintains and adjusts schedule to enhance department performance.
  • Effectively demonstrates the mission, vision, and values of the agency on a daily basis
  • Maintains confidentiality.

Requirements:

  • High school diploma or the equivalent
  • 1 year experience in a health care related role, preferably home health
  • Experience in insurance and health care billing preferred.
  • Experience in electronic medical records preferred.
