Advanced nursing practice is defined as the performance of advanced level nursing practice activities that, by virtue of post basic specialized education and experience, are appropriate to and may be performed by an APRN.

(Conn. Gen. Stat. 20-87a (b)).

For the first 3 years after having been issued a license and no less than 2,000 hours of practice, NPs must perform acts of diagnosis and treatment of alterations in health status in collaboration with a physician.

The collaboration must address a reasonable and appropriate level of consultation and referral, coverage for the patient in the absence of the APRN, a method to review patient outcomes and a method of disclosure of the relationship to the patient.

The collaborative agreement must be in writing and include the level of schedule II and III drugs that the NP may prescribe and a method to review patient outcomes.

Essential Functions & Responsibilities:
Pediatric or Family Nurse Practitioner provides clinical care within scope of Nurse Practitioner license.


Essential Job Functions:

Assesses patient illness by obtaining complete medical history ensuring that findings are properly recorded in the patient's medical record. Performs physical examinations, orders immunizations, orders laboratory tests/diagnostic imaging as needed and interprets results.
Diagnoses common childhood illnesses or health needs based on interpretation and evaluation of clinical data and physical assessments.

Works with other staff to plan, develop and evaluate patient care plan and advocates for and supports community health programs and policies.

Provide direct patient care as a member of the clinical team working with

Nurses, Providers, Behavioral Health Clinicians, Medical Assistants, Case Managers and Patient Access Representatives toward achieving the goals for the patient's plan of care.

Concentration with pediatric/adolescent population as appropriate to skills and Board certifications.
Participates in rotation to provide services to the community and participate in health fairs as needed.

Provide patient care and treatment in collaboration with a physician in accordance with established state laws and organizational job description.

Refers patients for diagnostic tests and specialist services as appropriate.
Participates in Professional Practice, Quality Improvement, Staff, and other meetings as assigned.
Maintains and enhances professional development and knowledge of theory and practice by attending conferences, educational programs and seminars.
Facilitate educational programs for pediatric or adolescent population.
May work in clinic setting based on operational needs.
Other duties as requested.


Performance Goals:

Ability to problem solve and use good judgment on a case-by-case basis.
Ability to effectively track client data, specialty appointments and diagnostic tests.
Ability to effectively assess patient status.
Aware of resources, referral options and services outside of the CHWC


Additional General Requirements:
Professional positive attitude, understanding of customer service principles, trustworthiness and excellent interpersonal skills.


Job Qualifications/Requirements:
Current APRN license. CPR certification. Mandatory Education, Case Review, and Departmental Meetings as outlined by the Medical Director.


Education:


Position requires a master's degree from an accredited program in nursing and a successful completion of an accredited certified nurse practitioner program.


Experience:
This position requires experience as a nurse practitioner with pediatric primary care experience.


Language Skills:
Must speak, write, and read English proficiently. Spanish preferred but not required.

Licensure/Certification Required:

A current license from the Connecticut Department of Health, Professional Board Certification, CT Controlled Substance Abuse and DEA License and certification in CPR.
