Starting Hourly Pay:
$18.42


Standard Employment Requirements

  • Must possess a valid and unrestricted Arizona driver's license (within 30 days of hire if applicant is from outofstate) with no DUI/DWI or reckless driving convictions in the last five years. No more than two atfault accidents in the last three years. Must maintain a valid Arizona driver's license during employment. Must be insurable under NATIVE HEALTH's liability auto policy.
  • Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  • Must pass substance abuse testing upon employment and submit to random testing during employment.
  • Must provide an updated immunization (IZ) record at the time of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  • Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.


The Community Health Worker (CHW) with the Arizona State Maternal, Infant, and Early Childhood Home Visiting Program (MIECHV) program provides assistance and support in obtaining health related services for clients of NATIVE HEALTH and the Prenatal Clinic as well as Women, Infants and Children (WIG).

The CHW is a liaison between the Arizona Department of Health Services, the health care provider, and the Phoenix Indian Community.


KEY ACCOUNTABILITIES:


  • Deliver services primarily through prescheduled home or office visits and scheduled health related classes.
  • Identify and enroll pregnant and postpartum women, caregivers, and their children in the program in accordance with the Arizona MIECHV policy and procedure manual.
  • Participates in training identified to be consistent with program objectives to provide education to benefit enrolled participants.
  • Documents and keeps records of all visits and case management services provided to families.
  • Supports the organization as needed.

Qualifications QUALIFICATIONS/EXPERIENCE/REQUIREMENTS**:


  • CHW must live in the identified neighborhood/community and reflect the characteristics of the community.
  • High School graduate or GED equivalency.
  • Has at least 23 years teaching experience in women's & children's health.
  • A sincere desire to improve the health status of the urban American Indian population and NATIVE HEALTH's community.

KNOWLEDGE, SKILLS AND ABLITIES:


  • Knowledge of the cultural and social background of the Native American and other clients of the NATIVE HEALTH service population.
  • Proficient in computer essentials, such as Microsoft Office (Outlook, Word, Excel, or Power Point).
  • Strong attention to detail.
  • Ability to adjust and be open to change as well as model change within the organization.
  • Ability to perform and provide excellent customer service to families, community, employees, and the overall agency.
  • Ability to meet the performance goals and contractual requirements of the Arizona MIECHV Program.
