Occupational Therapist - Skilled Home Health
SCHEDULE
Visits: 6–8 home skilled visits/day (minimum 6 points/day)

REQUIREMENTS
- Minimum 1 year OT experience required; 2 years preferred
- Must hold a valid State OT license (FL license or compact license required at time of submission)
- BLS certification required
- OASIS training preferred
ROLES & RESPONSIBILITIES
- Provide therapeutic interventions to adults and geriatric patients in a home health setting.
- Assess patient needs and develop individualized treatment plans to achieve specific goals related to daily living activities.
- Educate patients and families on conditions, treatments, and home exercise programs.
- Document patient progress and modify treatment plans as necessary to ensure optimal outcomes.
- Collaborate with interdisciplinary teams including Physical Therapy, Social Services, and Nursing.
- Ensure compliance with regulatory standards and organizational policies.
COMPLIANCE / NOTES
- COVID19 vaccine required (medical/religious exemptions only)
- Flu vaccine required or declination; masks mandatory while at work
- Valid driver's license and personal vehicle required for home visits
- 100mile radius rule applies (traveler must live at least 100 miles from facility)
- Compliance documents due two Fridays before start (10 days prior)
