POSITION SUMMARY:
As a part of the Food is Medicine Coalition, Project Open Hand is a leader in delivering nutrition interventions to help individuals manage chronic diseases that are nutrition sensitive. Recently, POH has been partnering with managed care plans to delivery medically tailored nutrition interventions through newly expanded MediCal benefits CalAIM Community Supports. This Registered Dietitian Nutritionist (RDN) position works in the Nutrition Services Department to provide nutrition assessments, counseling, and education to Project Open Hand clients living with critical and chronic illnesses, with a focus on Community Supports contracts. The Nutrition Services Department is an integral part of the Community Supports contract that includes the application of Medical Nutrition Therapy via in person and telephonic visits to some of the most vulnerable in our communities. The RDN develops nutrition education materials and curricula, assesses the needs of the client community, and adapts programming and medically tailored meals as needed to meet the unique challenges faced by Project Open Hand clients.
This position is ideal for a compassionate, professional dietitian who would like to help continually improve programs that sustain our clients as they battle serious illnesses, food insecurity, isolation, or the health challenges of aging. This position requires a level of comfort in the community nutrition environment with an emphasis on education provided with a trauma informed approach to nutrition care.
This position is intended to be a hybrid position working out of our Oakland office, and remotely. Fully remote may be a possibility for some candidates.
QUALIFICATIONS REQUIRED:
• Registered Dietitian, Registered Dietitian Nutritionist
• Excellent verbal and written communication skills
• 1+ year experience working with people with chronic health conditions
QUALIFICATIONS DESIRED:
• 1+ year of community nutrition-based work experience
• Experience working with people living with HIV/AIDS and other critical and chronic diseases
• Experience developing and executing community based nutrition education classes
• Experience working with vulnerable populations
• Bilingual in Spanish
• Ability to apply strong clinical skills in a community-based setting
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
· Participates in intake of new clients, conducting nutrition assessment and follow up sessions for clients by phone or telehealth platforms for clients enrolled in Community Supports contracts.
· Document nutrition assessment and counseling sessions for each client utilizing the Nutrition Care Process
Provide nutrition education and/or cooking classes for clients focused on topics including diabetes, heart disease, and other topics as determined relevant.
• Regular participation in team meetings.
• Supervises dietetic interns as assigned.
