Nurse Practitioner

Monogram Health is seeking skilled Nurse Practitioners and Physician Assistants excited about helping patients improve their quality of life and slow the progression of kidney disease.

As an Advanced Practitioner at Monogram Health, you will work alongside a Care Team that includes a Registered Nurse and a Social Worker, making a significant impact on patients dealing with multiple serious diseases.

Your role involves delivering personalized medical care, maintaining accurate patient records, and ensuring positive health outcomes for individuals mainly with CKD and/or ESRD/ESKD.

Roles and Responsibilities:

Conduct assessments and wellness exams
Educate patients and families on available programs
Coordinate care and maintain quality standards
Deliver timely care to avoid hospitalizations
Prescribe medications and collaborate with physicians
Facilitate patient education and care plan adherence
Assist with preventive monitoring and gap closure
Participate in integrated care team meetings
Requirements:
Bilingual in English/Spanish preferred
Active, unrestricted RN/NP/PA license
Board certification and DEA certificate
Ability to work autonomously and communicate effectively
Proficiency with medical tools and technology
Knowledge of data management and medical records systems
Understanding of federal and state regulations
2+ years of direct patient care experience
Managed Care/IPA/Health Plan experience
Preferred: Advanced RN Practitioner or Nurse Practitioner certification
