Major Responsibilities:

  • Provides psychiatric services to patients of the community behavioral health center as assigned.
  • Completes psychiatric assessments, medication consultations, and follow-up services consistent with the advanced practice agreement with the supervising M.D.
  • Provides each patient, in collaboration with the clinical team, a comprehensive assessment, diagnostic formulation, problem list, treatment plan, and ongoing patient care, further evaluation, consultation and collaboration as necessary to address the safety and treatment needs.
  • Provides medication assisted treatment to individuals with an opioid addiction.
  • Works collaboratively as a member of the patient-centered Adult Outpatient clinic's multi-disciplinary open access teams to coordinate the delivery of appropriate clinical and medical services.
  • Maintains all documentation for services provided, with review of information from other providers in the Electronic Medical Record (EMR).
  • Works in the Adult Outpatient Services clinics in Worcester, Leominster, and Fitchburg.
  • Prepares and maintains confidential client documentation, service records, staff activity logs (SALS)
  • Demonstrates knowledge base of adults with severe and persistent mental illness, chronic substance use conditions, and medical conditions.
  • Maintains current knowledge of psychiatric medications.
  • Accurately assesses client's presenting concerns.
  • Effectively communicates with clients, peers, and other professionals.
  • Timely and accurate documentation skills.
  • Performs other related job duties.

Position Qualifications:

License/Certification/Education:

Required:

  • Licensure as a Psychiatric Clinical Nurse Specialist by the Massachusetts Board of Registration in Nursing.
  • Current DPH and DEA certificates.
  • Has an active X Waiver or on track to obtain the waiver.
  • Meets all qualifications for the advanced practice of psychiatric nursing with prescribing privileges.

Experience/Skills:

Required:

  • Experience working with adults with severe and persistent mental illness, substance use disorders, and co-occurring developmental disabilities and/or medical conditions.
  • Must be able to pass a CORI background check.


Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

  • Must be able to move about the space periodically during the shift.
  • Must be able to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medicals codes or alarms).
