Description
We are seeking a compassionate and experienced Hospice Nurse to join our team. As a Hospice Nurse, you will provide specialized care and support to patients and their families during end-of-life stages. This is a rewarding opportunity to make a difference in the lives of patients and their loved ones.


Duties:


  • Provide comprehensive nursing care to hospice patients, focusing on pain and symptom management, emotional support, and comfort care
  • Conduct patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed
  • Collaborate with interdisciplinary team members to ensure holistic care for patients
  • Educate patients and families on disease progression, treatment options, and endoflife care
  • Assist with coordination of services, including home health aides, LVN, social workers, and chaplains
  • Maintain accurate documentation of patient care in electronic medical records (EMR) system (Kinnser)
  • Monitor patient condition and report any changes to the healthcare team
  • Provide emotional support to patients and families throughout the hospice journey

Experience:

To be successful in this role, you should have the following qualifications:

  • Minimum of 1 years of nursing experience in hospice setting
  • Strong knowledge of infectious disease care and senior care principles
  • Proficiency in using electronic medical records (EMR) systems, specifically Kinnser
  • Experience in acute pain management and trauma medicine
  • Ability to perform patient assessments and develop appropriate care plans
  • Familiarity with botulinum toxin treatment for symptom management
  • Competency in specimen collection processing and laboratory procedures

Pay:
$ $39.00 per hour
