The Community Integration Specialist will play a key role in promoting the social, emotional, educational, and vocational wellbeing of individuals with diverse backgrounds and abilities.
This position involves working closely with clients, their families, and community partners to develop and implement individualized plans that support successful community involvement.
Key Responsibilities:
- Conduct assessments to identify clients' strengths, needs, and goals related to community integration with a priority focus on mental health
- Develop personalized service plans in collaboration with clients and their support networks.
- Facilitate access to community resources, services, and opportunities.
- Advocate for clients' rights and needs within the community and with service providers.
- Build and maintain strong relationships with community partners, service providers, and local organizations.
- Monitor and evaluate clients' progress towards achieving their integration goals.
- Maintain accurate and uptodate client records and documentation.
- Participate in team meetings, training sessions, and professional development opportunities.
Qualifications
- Minimum of Associates degree in social work, human services, psychology, or a related field.
- Must be eligible for MHRTc
- Strong understanding or willingness to learn community resources and the ability to navigate and connect clients to these resources.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Demonstrated ability to advocate effectively for individuals with diverse needs.
- Valid driver's license and reliable transportation.

Pay:
From $52,000.00 per year
