    Description

    The Senior Case Manager assigns duties to case management staff and manages their work product pertaining to performing tasks related to their caseload assignments. The Senior Manager uses his/her technical skills and knowledge to analyze and recommends strategies for work performance improvement. A senior case manager also directly provides comprehensive case management services for adults with serious mental illness or children/adolescents with emotional disturbances (and their families) to improve quality of life and maintain the highest possible ability to recover and function within the community. This position promotes recovery-based, person-centered, and culturally competent practices. This position requires working in the community and performing home visits.

    Leadership

  • Mentor/Coach individually, and as a Team, all assigned case managers.
  • Provide Training, Shadowing. and Onboarding to new case managers assigned to your team.
  • Meet weekly with case management staff, addressing clinical issues.
  • Promote a recovery centered environment and provide referral to appropriate community resources
  • Act as a consultant to assigned case manager team members-pertaining to the delivery of day-to-day clinical services, report engagement to clinical program manager (timelines based on program) and escalating areas of concern to Clinical Manager, as needed.
  • Identify emergency crisis. Make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques: coordinate other services.
  • Case Management

  • Ensure consumer involvement in the Person Centered Planning Process (PCP).
  • Facilitate the implementation of Individual Plan of Service.
  • Perform comprehensive, ongoing psychosocial assessments.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Maintain compliance with assigned caseload with person centered treatment plans, maintain % or higher on quality of care audits from assigned caseload, meet all required timelines for documentation, maintain a high level of case management requirements and ability to provide oversight of team members.
  • Utilize his/her technical skills and knowledge to analyze and recommends strategies for recovery.
  • Coordination of Care

  • Facilitate the coordination of consumer intakes, discharges, and transfers.
  • Communicate with all service providers in Treatment Plan.
  • Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
  • Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.

  • Bachelor's degree in a human services (Social Work, Psychology, Sociology, Criminal Justice...) field from an accredited institution required.
  • Master's degree preferred.
  • LMSW Preferred
  • Qualified Mental Health Professional (QMHP) and/or Qualified Intellectual Disabilities Professional (QIDP): Individual with specialized training or one year experience in treating or working with a person who has mental illness and/or intellectual disabilities; and is a social worker, registered nurse, licensed or limited-licensed professional counselor.
  • Knowledge of the following areas preferred: Concepts of Motivational Interviewing, Integrated Duel Dual Diagnosis Treatment, Family Psycho Education, Stages of Change, and Trauma Informed Care.
  • One year of experience treating or working with SPMI and/or I/DD population.
  • Two years case management experience with adults.
  • Valid Michigan Driver's License.
